Service Authorization

The following links provide forms and information regarding service authorizations issued by the Public Service Commission. An attorney licensed to practice law in Missouri must file the forms identified below. 

Telecommunications or Interconnected VoIP Service

• Application Form - This form is used to initially provide service (e-Filing Instructions*).
• Telecommunications Notice of Change Form - This form is used to subsequently revise a telecommunications provider’s authorized service area (e-Filing Instructions*).
• IVoIP Notice of Change Form - This form is used to subsequently revise an IVoIP provider's authorized service area (e-Filing Instructions*).

*The linked instructions are designed for VoIP; however, with slight modifications the instructions can also apply to telecommunications certificate applications.
