Statement of Revenue
Gross intrastate operating revenue is to be reported in compliance with the provision of Section 386.370 RSMo. A pre-printed, company specific Statement of Revenue form was mailed to all certificated utility companies and registered IVoIP companies on January 19, 2011.
An Adobe fillable document and instructions for completing the form are also available on this website under “Forms”. The calendar year 2010 Statement of Revenue must be filed with the Commission on or before the due date of March 31, 2011.
