2012 Statement of Revenue
Gross intrastate operating revenue is to be reported in compliance with the provision of Section 386.370 RSMo. A pre-printed, company specific Statement of Revenue form was mailed to all certificated utility companies and registered IVoIP companies on January 18, 2013. An Adobe fillable document and instructions for completing the form are also available on this website under “Forms”. The calendar year 2012 Statement of Revenue must be filed with the Commission on or before the due date of March 31, 2013.

The completed Annual Report form shall be received by the Missouri Public Service Commission on or before April 15, 2013.
Request for Extension of less than 30 days:
To file a Request for Extension of less than 30 days, you must make your request on or before April 15th. All annual report extension requests must be on company letterhead and signed by an officer of the company or on the form provided below and signed electronically. You must include an email address for notification of approval or denial of the extension request.
