ORDER RECOGNIZING NAME CHANGE

 

Issue Date:  October 28, 2014                          Effective Date:  November 20, 2014

 

 

On October 20, 2014, Value-Added Communications, Inc. asked the Commission to recognize its name change from Value-Added Communications Inc. to Value-Added Communications, Inc. d/b/a GTL.  The company also filed a revised tariff title sheet to be effective November 20. 

In response, the Commission directed its Staff to file a recommendation regarding that request by November 3.  Staff’s recommendation, filed on October 24, advises the Commission that the notice of name change and the revised tariff title sheet comply with the Commission’s requirements for a name change. Staff recommends the Commission recognize the name change and either approve the tariff or allow it to go into effect. No one opposes the name change.

The Commission has reviewed the notice of name change and Staff’s recommendation. The Commission finds that the name change should be recognized.

THE COMMISSION ORDERS THAT:

1.               The Commission recognizes the change of name of Value-Added Communications, Inc. to Value-Added Communications, Inc. d/b/a GTL.

2.               The revised title sheet filed by Value-Added Communications, Inc. d/b/a GTL is approved to be effective on November 20, 2014.  The tariff sheet approved is:

P.S.C. MO No. 2, Cancels P.S.C. MO No. 1

First Revised Sheet No. 1, Cancels Original Title Sheet No. 1

 

3.               This order shall be effective on November 20, 2014.

4.               This file shall be closed on November 21, 2014.
